Dear JET members,

The current health insurance card will become invalid on December 2, 2025.
The attached is the notification for employees issued by Minoh City Office.

The HR has sent 資格証明書(health insurance eligibility certificate; a card 
very similar to the health insurance card, but it has not 健康保険証 on the 
card but 資格確認書 on it) to your schools showing that you are qualified as 
a health insurance union member.

According to the HR, the 資格確認書 has already been issued to the JET 
members who started service this year.
If you have not yet registered your individual card(my number card) for 
health insurance, you should bring the 資格確認書 with you when visiting 
clinics.

You do not need to return your current insurance card to the HR, but you 
should return the document when you begin using your individual card for 
health insurance or when you leave the JET program.
Please check that the information on your 資格確認書, including the symbol, 
number, name, birth date, and sex, is correct.

If you find any corrections, report them to the HR.
The current document will be valid until November 30, 2029.

any questions regarding this matter.
