Hi, Minoh JET-ALTs,

This message is to notify you of the “Emergency support of cash handout 
scheme by Minoh City (70,000 yen)”
Letters have been already sent to households, which match the following 
conditions;
-Those who live in Minoh City on December 1st, 2023
-The residence tax of 2023 is not taxed

The following are advices of how to fill in the form in accordance with 
the number described on the application document.
1    (1) Write your telephone number
      (2) Write the date you filled in and your signature next to the date
      (3) Write name of bank, branch, type of account, account number 
and account owner
If bank account is already printed on the document and its information 
are not amended, you do not need to fill in (3).

２ ・Attach a photo copy which identifies you, example: residence card, 
driver’s license, passport or my number card.
　・Attach a photo copy which show your bank account, example :bank book, 
cash card or screen copy of internet banking.(not necessary if bank 
account is already printed on the document and its information are not 
amended)

３　・Detach the document, put it into envelop enclosed and mail it. (No 
necessary to put a postal stamp)
