Minoh JET ALTs,

I hope this email finds you well.

Regarding the printed name on your health insurance card, your first 
name is not shown on the card in most cases.
This seems to be caused by the mistake in handling the paperwork of the 
HR dept. of Minoh City Office.

We found out that the name can be fixed by filing extra paperwork.
If you wish to fix it, please come to JET House Room #101 during MAFGA 
stationing hours(Wed&Fri 5-7pm).
Please bring your current health insurance card.

Although the current health insurance card is still valid even your 
first name is not on, we hear that you have to explain the reason why 
the name is different in some cases.
It may be better to fix it to avoid trouble in the future.
